The next lithium dose was held and his daily dose was reduced to 1500 mg.  A repeat level on 3/25/98 was 1.3 m#1/L and the patient's mental staus was improving well as his gait.  It is important to note that the patient's signs and symptoms of mania were resolving over the week prior to this elevated level.  The elevated lithium level could be explained by a change in lithium clearance due to a change in affective status
